Layu
Aku lihat layu dalam Melayu
Aku pasti bukan aku seorang saja yang nampak
Ada juga bercadang bertanya tuan punya
Tapi bercampur risau dengan tak tergamak
Nanti dikata tak bersyukur, dikata tak sedar diri
Kita siram, kita baja tapi angin api kita lupa
Mereka merancang masa menjilat bangsa
13 harimau bertukar menjadi mangsa
Kita lupa nenek moyang kaya-raya
Jangan sekali-kali digadai harta-bendanya
Ku lihat layu dalam Melayu
Ku lihat layu dalam Melayu
Ku lihat layu dalam Melayu
Takkan Melayu hilang di dunia
Ya... tapi apa guna tak hilang di dunia
Kalau kewujudan tidak dirasa
Petah berbahasa kudrat tak berjasa
Orang berbudi kita hanya tahu merasa
Selalu lari bila dirapat
Selalu malu bila soalan diaju
Selalu segan memberi pendapat
Rela mengikut dari meneraju
Belum nyanyi sudah bersorak
Suka berjanji dalam borak
Bukan kata tak ada otak
Cuma tak berfikir di luar kotak
Ku lihat layu dalam Melayu
Ku lihat layu dalam Melayu
Ku lihat layu dalam Melayu
Aku jadi sayu
Bunga menangis dia mendayu
Mahu jadi cantik tetapi ragu
Aku merayu
Bangunlah semula hidup kembali
Jangan tunggu matahari mati
Cepatlah mekar sebelum terlerai
Perjuangan kita belum selesai
Perjuangan kita belum selesai
Perjuangan kita belum selesai

Adapting (Malay's not Malay)
What i want to highlight here is regarding some practice done by Malay which is actually no longer can be practice today. It is outdated. Not to say i am to modern on this but it is a simple thing and with little effort to think, you might get my point here.
The first thing is regarding toilet paper. How many Malay use toilet paper? Count them by your finger. I do believe that malay sometime don't want to change. I was kind of annoyed by some Malays who simply classify a malay using toilet paper as non-malay. The saying of " Toilet paper? u want to be chinese ka?" or " Not clean lar, only chinese do that" is common.
What i discover is, toilet paper is not created for the chinese nor western people or people who are unhygiene. It is a hygiene product. I learn this by using only 0.0001% of my brain.
Simple question: Which one is cleaner? 1. People using water and hand to clean their **** or people who are using toilet paper and water?
Can anybody just think about this? Obvious.
" Who told you to wipe your *** with a toilet paper and forget to use the water? That is unhygiene. But to use both, is much more hygiene.
This is because, your hand will be contaminated when you use it to clean your butt. How can you say this is the most hygiene practice? It is not. Even though you clean your hand with those soap product that claimed to kill 99% of bacteria in your hand, it will not clean your hand effectively. So to clean it is to prevent your hand from touching the most contaminated area in your body.
Another thing is about eating with hand. Most of Malay would scold their children or maybe classified someone as non-malay, or 'lupa daratan" if a malay eat with spoon and fork.
The saying " You already forgot your race ka? You are Malay, use hand la" or " Kacang lupakan kulit sudah, makan pun guna sudu" are common.
The problem is, which one actually is much better. Using your hand that contains numerous bacteria ( for your infoormation, our hand contains a million of normal flora a.k.a bacteria that cannot be removed by cleaning using soap or alcohol. They are our friend) or using a spoon and fork which is hardly have normal flora. Yes spoon and fork can contain bacteria but compare it to our hand, too obvious that our hand cannot be so clean because it contain moisture and media for those bacteria to live, but spoon and fork do not. Bacteria on the spoon is often contaminated from other outer source. But our hands have their own population grwoing for generation.
So eating using utensils is much more hygiene that hand. But Malay would always reject this tradition.
However, some practice of Malay cannot be denied. For example, squatiing toilet. Okay. This is funny. By rejecting the two point i have discussed above, Malay today are adapting to the sitting toilet so rapidly. They did not care of this change. They would say that sitting toilet is much better than squatting. Squatiing is old timer.
They did not realize that physiologically, squatting toilet is the better toilet for us. Squatting is a position which bring benefits to our body. By squatting, you can increase venous return to your heart. You will increase intrabdominal pressure which will help in passing your faeces out from the anus. You don't have to strain a lot when you squat. Another thing is for the girls. Do people know that the best position for delivering a baby is by squatting? Yes. this position is still adapted now in Obstetric practice during delivery. You may not realize that McRobert position that is use if the baby shoulder is stuck is actually a squatiing position on the bed. And this position is often teach to pregnant mother so that in emergency case like that, they can quickly do this position and deliver the baby safely.
Most of western women do not know how to squat, and this is difficult for them when they are pregnant and some obstructed labour happen.
So squatting is physiology. But i do not know why, Malay are trying to abandone this practice. Not only Malay, all other races too. A consultant in Hospital Likas here in Kota Kinabalu once said that the hospital is upgrading the toilet to become sitting toilet but opposed by him because for him, a obstetric ward should have only one toilet which is squatting toilet.
As a conclusion, i am not a person who opposed things blindly like someone else did. For toilet paper, you have an option to use it or not. If you want, you get the benefit, if you don't, at least you wash your ass with water and washed your hand with soap, still no wrong in that. For spoon and fork,, i would never ask people to abandon the technique of eating by hand. Even i would teach my children and future generation to use hand to eat because i don't want them to dependent on utensil. If you stuck somewhere without spoon, you stillc an eat. For toilet issues, i guiess it is your option to use squatting or sitting. Sometime, old people prefer sitting because squatiing make their knee ache. So it depends on the situation. I believe my parent are so clever because my house have 2 toilet, one is sitting and one is squatting. We have choice to use which one. So in the end, unless you do no harm, you are free to choose. But once you burst out racist statement like "toilet paper is for chinese", you are causing harm, and you must start to use your brain in the future to make sure that no more harm can be done by you, to you, others, and your future generation.

Sahabat Kartika
Jika direnung kembali, perlukah orang sebegini dijadikan idola?
Entah. Siapa tahu?
Listening story
Different patient will gave different history. It is nice to know what others experienced in their life.
Sometimes during on call, i like to talk to some patient not regarding their medical illness but about other thing, their story. I like to listen to them and sometimes this is therapeutic as telling someone your story can relieve your pain.
Somehow, i start to realize the importance of history taking. It can make someone comfortable. You will feel comfortable if you tell other regarding your problem. Telling someone who is willing to listen.
A good speaker is the one who listen.
By this statement, do you realize that there is a lot of bad speaker in this world. When you look at all the people in the media, they talk, they talk brilliantly, you cannot beat them in their manifesto, fact and the flow of their talking. When they meet people, they mixed well with other quickly, people start to think that this kind of people is reliable. However, in the end, people would say " dia tu cakap kosong ( only know how to speak without making thing real)". Why this happen? because this people don't know how to listen.
Listening is much more powerful than speaking. By listening to other, you can control them easily but by talking non stop to them, the rate of people comply with you is lower.
Lstening skill also important in our life. When someone talk, we listen. Wait until there is dot in their line than you can start to speak.
Do you realize since our grandmother's era that lot of people hate people who do not listen?
These people are called 'degil'. Somehow, since a long time ago they realize that you must know how to listen and not only talk.
So now, everytime someone talk to me, i try to listen to them. Honestly i am also not a good listener. But i will do my best.

Patient Patient.
was going to clerk this patient and that patient agree to be clerk by
me. Patient autonomyis my first priority. However, suddenly this one
relative (maybe the son of that patient) suddenly acting clever and
tell me " If you want to ask questions, ask the doctor, not the
patient, can you see this patient is sick, why must you ask
question?".
Fair enough, i don't want to start a fight, we are professional, thus
i had to leave that patient but more spesifically the relative. The
wife of that patient already agree with me, and the patient also. I
guess patient autonomy does not apply for their relatives.
Fair enough, we still need to respect our patient, and we need to obey
them. if they refuse their medication also no problem nowadays because
patient have their autonomy.
What i want to highlight here is regarding 'asking question' or in
medicine it is called history taking. We could not say in malay "
ambil sejarah" because everyone will be blurred with that term.
History taking is one superior tool in making diagnosis in medicine.
Layman would picture doctor to read x-ray, holding stethoscope,
holding needle, and handle sophisticated modern machine to diagnose a
disease. However that is not the most important tool in medicine.
History of the patient is much more important.
More often drama and tv shows are showing doctors like house and
Grey's anatomy doing boombastic medical procedure to diagnose their
patient but to tell the truth, the part where they took history was
cut and they only show important question asked by them. So it would
not be boring.
In reality, we are spending at least 20 minutes to talk to the patient
and to ask them everything regarding their problem.
This is the first sentence that i heard when i enter medical school.
"80% of the diagnosis are made from history taking".
So asking question to patient is so important until all medical
doctors, i am sure, from everywhere in the world, they would need to
prepare themselves to be good in taking history before they can become
a good doctors.
However, most of patient and their relatives, did not understand that
history taking is used to diagnose patient. They think doctor just ask
question and talk with them without any purpose. I remember a friend
telling me that he read in the newspaper regarding a complain from
someone who complained that a doctor was insulting her father because
that doctor asked about her father occupation. She said that her
father was obviously poor by the way he dress thus why that doctor
need to ask about occupation some more.
It was a blunt with no guide complain. Occupation (social history) is
so important so that you are not only looking to the financial status
but more on to look what actually patient do in their work. poor man
can be sitting in the street begging for money, and poor man can also
work by lifting 200 pounds of wood on his shoulder everyday. If both
of this poor man present with inguinal hernia. is it the beggar or the
heavy lifter would be predispose to recurrent after treatment?
Doctors are looking beyond the scope of the disease. We take care of
patient not only in hospital but after discharged. Where the patient
would go, are the patient to continue his old job.. everything.
So, i hope that people would understand that asking question is
important in diagnosing a problem. It is common sense.
Doctors are people who want to knoe and solve the problem. So they
asked question. And they need to know.
p/s: A long time ago. when i was in secondary school. a friend tell me
a story about his grandfather. the story goes like this.
His grandfather would not talk a single word when he came to see a
doctor. When the doctor ask what is his problem, he would scold the
doctor by saying " you are the doctor, then you are the one who need
to tell me what is my problem!".
that time, i believed that that old man is right. But time change as
we became wise, now i would clearly and firmly said that he is totally
wrong. Doctor would not know wnything if you did not tell. History
taking contribute 80% of your diagnosis. Thus, if you did not tell,
doctor can only make it 20% or less. Tomorrow you will end up with
more catastrophic consequences.
